1. A computer with
Internet access and a web browser (Internet Explorer, Netscape,
etc.).
2. Adobe
Reader (Adobe
Acrobat Reader) version 5.0 or greater. Please note that this
software is available for free for Windows, MacOS, and Linux
users. Visit

3. If you want to
e-mail a completed form to a client, you will also need the
free PDF995 software printer. This simple
software printer allows you to save a completed form to a file
and e-mail the form to a client. The client can then open the
file using Adobe Reader, print the form, and fax or mail it

Completed Forms to Clients). An alternative to the PDF995
is the CutePDF
Writer. It is a free pdf software that can be used to save
completed forms. Visit the CutePDF
